Step 1:

Please write your name and collection date on the specimen vial. Write your date of birth on the specimen vial as well where it says “I.D. Number”. If possible, void urine prior to collecting stool. Put gloves on and pass stool into collection tray.

Step 2:

Do not discard the pink liquid in the specimen vial. Using the spoon attached to the cap of the specimen vial, spoon stool from different areas of the sample into the vial (try to collect from at least 4 different areas of stool in collection tray, going left to right). Fill specimen vial to the red fill line. Carefully mix stool & fluid with the spoon attached to the cap. Replace cap tightly & shake vial vigorously for 30 seconds.

Step 3:

Place capped specimen vial containing the collected stool sample into the Zip Closure specimen bag along with absorbent pad. Seal the bag. Fill out the “Test Request Form”: The only areas you need to fill out are 1) Patient Information (fill out the entire box) and 2) Date of Sample Collection (which is found under “Test Information” box). Leave all other areas blank, as the lab will be billing Alexa. Place “Test Request Form” into the document holder of Zip Closure specimen bag. Place the Zip Closure specimen bag into kit box.
